Job Description

At McCormick Post Acute, a trusted long-term care provider in South Carolina, we are committed to excellence in person-centered care. We’re seeking an experienced full-time Social Services Assistant who will play a vital role in supporting the emotional and psychosocial well-being of our residents. Key Responsibilities include assisting in the development and implementation of resident-centered social services programs; supporting residents and families throughout the admission, care, and discharge process; maintaining timely and accurate documentation in compliance with South Carolina DHEC and federal regulations (including CMS); participating in interdisciplinary care planning to address resident needs and preferences; responding to resident concerns and promoting their rights and dignity; assisting with MDS 3.0 assessments and care plans as appropriate. Additional duties involve assisting the Social Service Director/Consultant in planning, developing, organizing, implementing, evaluating, and directing social service programs; meeting with administration, medical and nursing staff, and other departments; reviewing and recommending changes to department policies; assisting in identifying medically related social and emotional needs of residents; participating in community planning; assisting residents in achieving the highest practicable level of self-care, independence, and well-being; providing medically related social services; performing administrative requirements; providing information on Medicare/Medicaid and financial assistance; consulting with staff and community agencies; maintaining social service progress notes; coordinating social service activities; working with emotional problems of residents and families; evaluating social and family information; assisting in identifying and correcting problem areas; obtaining resources from community agencies; assisting in outpatient appointments and ancillary services; assisting in discharge planning; developing and maintaining good rapport with other departments and community agencies; assisting in orientation and in-service training programs; assisting in budget development; reporting suspected fraud; maintaining confidentiality of resident care information; and reporting unauthorized disclosures. The position requires a minimum of a high school diploma or equivalent, with associate or bachelor's degree preferred but not required. Prior healthcare experience is preferred. The role demands strong interpersonal, organizational, and documentation skills, knowledge of South Carolina DHEC regulations and CMS standards, and the ability to read, analyze, and interpret business and governmental regulations. Physical demands include occasional lifting of up to 25 pounds, prolonged computer use, and the ability to sit, stand, walk, talk, read, or hear regularly. The work environment is typically low to moderate noise level. Reasonable accommodations may be made for individuals with disabilities.
